Initial Visit Report for Care Coordination Payment Changes Webinar

OHCA and DXC will be presenting a SoonerCare/Insure Oklahoma Medical Home Providers Webinar on Thursday, February 15, 2018 at 2:00pm. Effective January 1, 2018, the Oklahoma Health Care Authority (OHCA) only pays care coordination fees for members that have had a visit with their assigned provider.  On a monthly basis prior to the care coordination payments being calculated, OHCA will look back 15 months to make sure you receive care coordination for all established members aligned with your practice.  This webinar will cover a new report located on the provider portal which displays members who are at the 15 month time frame as well as those who are within 45 days of hitting 15 months. We will show how to access the report and explain the format of this new report for Medical Home Providers. This webinar is recommended for SoonerCare/Insure Oklahoma Medical Home Providers.
