Publications Ordering

We offer several free publications that feature various SoonerCare programs. These publications may be downloaded individually or visitors may place an order for a quantity to be delivered to them. Orders are processed every Thursday. Please allow three weeks for delivery.

Step 1. Type in the number of copies you wish to order for each title.
Please order items in multiples of 10. Specify individual item count, not by number of cases.

ApplicationsView/DownloadOrder Quantity
The SoonerCare Health Benefits Application. (English) (Previously SAPENG–SCAPPL & SAPENG-SC1APP-2007) SC-1 As per agreement with OKDHS administration, OKDHS county offices are supposed to print the applications as needed from the PDF located on the website. They are not supposed to order the applications from this site.SoonerPlan application for Family Planning Services. These applications have now been added to the regular SoonerCare applications.
